Your First Visit

Appointment Information

On the day of your first visit please arrive 10 minutes early to allow time to fill out the necessary paperwork. Your first visit will last approximately 50 minutes. It will include a history, a physical examination, discussion of findings and a treatment plan. If time allows, treatment may also begin on that day.

Payment Information

We accept cash and personal checks. We do NOT accept credit cards.

We do not accept insurance.
Upon request I can provide you with a fee slip that may be submitted to your insurance company which may be reimbursed under their out-of-network guidelines.
Payment is expected at time of service.
